29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a): Exterior - Site - The employer did not instruct each employee in the recognition and avoidance of unsafe conditions related to operating rock drilling machines. Employees were exposed to caught-in and amputation hazards while operating the Rock Commander drill. An employee suffered a finger amputation when his hand was caught inside the drill's centralizer, on or about November 17, 2025.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
