Site - Employees were exposed to stuck by hazards when a large storage rack system holding boats was constructed in a manner inconsistent with manufacturers installation instructions and the requirements of  ANSI 16.1-2023: Industrial Steel Storage Racks which compromised structural integrity and prevention of collapse from, including but not limited to, sway, seismic activity, and fork truck impact.

Site - Employees were not protected from struck by hazards when a beach umbrella was secured to the roof of a Doosan D90S-7 forklift, hindering the overhead visibility between the operator and the load.
