On or about November 10, 2025, an employee fell from the form bracket scaffold and was impaled on rebar.

29 CFR  1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.   Site- An employee working from a form scaffold was not protected from a fall hazard where the scaffold was not inspected by a competent person prior to use. On or about November 10, 2025 an employee fell from the form bracket scaffold and was impaled on rebar.
