29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  a) New residential construction located on 2085 Octave Place, Cumming, GA 30040: On or about 11/24/2025, the employer failed to ensure that scaffolds were erected and inspected under the supervision and direction of a competent person qualified in scaffold erection, in that employees were permitted to erect scaffolding without oversight by a competent person, and the scaffold was not inspected by a competent person prior to use. As a result, employees were exposed to fall hazards of approximately 17 feet, 7 inches to the lower level.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) New residential construction located on 2085 Octave Place, Cumming, GA 30040: On or about 11/24/2025, the employer exposed employees to fall hazards of approximately 17-feet, 7-inches to the lower ground level, in that a fall protection system was not provided to employees engaged in siding installation activities using a job made scaffold system.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  a) New residential construction located on 2085 Octave Place, Cumming, GA 30040: On or about 11/24/2025, the employer failed to inspect all electrical extension cords used to provide power to electric tools, in that an orange 25-ft. electrical cord was damaged and exposing the internal insulated energized wires. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.
