The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence.  (a) On or about September 9, 2025, an employee was exposed to amputation hazards when reaching into the die area/point of operation/feeder, on the Bliss C-35 (35 ton) part-revolution mechanical press, to remove stuck scrap when the feeder indexed the metal, without ensuring the feeder was de-energized and controlled prior to commencement of work.
