This condition exposed employees to struck-by and crushing hazards while moving pallets of hoses weighing 600 pounds with a forklift.

Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. a) On or about November 25, 2025, the employer did not ensure secure and stable storage that will not slide or collapse in that pallets were found to be broken and setting 6 inches to more than 12 inches beyond the edge of the pallet rack. These pallets include but were not limited to in the hose bay, ND2, ND 8, NC11, and the idler hose bay near the hose bay electrical panel. The employees were exposed to struck by hazards from the pallets and the hoses on the pallets which weighed approximately 600 pounds.

29 CFR 1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion. a) On or about November 20, 2025, in the weld shop, the employer did not ensure that employees used work rests that were adjusted to within one-eighth inch to the abrasive wheel in that no work rests were present on the Dewalt DW756 6in Bench grinder serial # 2017 50 L54125.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.) a) On or about November 20, 2025, in the weld shop, the employer did not ensure that a tongue guard was installed on the Dewalt DW756 6?inch bench grinder, serial # 2017 50 L54125, used by employees to sharpen tungsten electrodes. The grinder was operated with no tongue guard in place, exposing employees to the hazard of wheel fragments and flying particles.
