29 CFR  1926.451(e)(1):Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access:  Worksite located at 25 NE Tudor Road, Lee's Summit, Missouri- Employees were exposed to fall hazards  while climbing the legs of a walk-through scaffold. These legs are not symmetric and could lead to a fall.

29 CFR  1926.451(g)(4)(i):Guardrail systems were not installed along open sides and ends of platforms:  Worksite located at 25 NE Tudor Road, Lee's Summit, Missouri- Employees were exposed to fall hazards while working on scaffold platforms that did not have endrails installed on the openings. The employer also failed to utilize a midrail on platforms where the crossbracing had left too big of an area in which employees could fall.
