Deficiencies noted during the inspection included, but were not limited to, employees working near a wall opening did not use appropriate fall protection systems/devices, exposing them to fall hazards of up to approximately 15 feet.  b) Job site, 341 Butler Street, Etna, PA: On or about December 18, 2025 during construction activities, the employer did not ensure that a system of hazard identification and correction was implemented at the site. Deficiencies noted during the inspection included, but were not limited to, temporary wiring having outer jacket stripped and exposing the inner wiring, inappropriate wire splices and duct tape being used over wiring entering into an electrical panel.
