29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a. On or about December 2, 2025, at BLDG 3: employees were exposed to over 25- foot fall hazards while installing underlayment on a 5:12 pitched roof, in that a fall protection system, such as lanyards was not used.

29 CFR 1926.102(a)(2): The employer did not ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.   a. On or about December 2, 2025, at BLDG 3: employees were exposed to eye injury hazards while using pneumatic staple guns to install underlayment on a 5/12 pitched roof, in that eye  protection was not used.
