29 CFR  1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.  98 Shell Beach Road, East Haven CT: On or about December 3, 2025, the employees working from the aerial lift had not received the required fall hazard training before being exposed to falls up to 20 feet and 4 inches while performing tasks such as (but not limited to) condominium balcony refurbishment.

29 CFR  1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  98 Shell Beach Road, East Haven CT: On or about December 3, 2025, the employer did not ensure that employees who were working from an elevated aerial lift basket at heights of up to 20 feet 4 inches above the ground kept their feet firmly on the floor of the basket as they were climbing the basket's guardrail.
