29 CFR  1910.147(c)(6)(i):  The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  On or about December 3, 2025, at an establishment located at 7079 Guelig Road, Lena, Wisconsin, in the South Building, the employer did not conduct periodic inspections of their energy control procedures to ensure that the procedures and the requirements of this standard were followed. Employees performed servicing and maintenance tasks on equipment including but not limited to, DeWalt air compressor system and Heartland Fabrication band saw dismantler.

29 CFR  1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  On or about November 12, 2025, at an establishment located at 7079 Guelig Road, Lena, Wisconsin, in the South Building, employees conducting pallet dismantling on the Heartland Horizontal Bandsaw were exposed to struck-by hazards created by bandsaw blade. The point of operation was not effectively guarded to prevent exposure to these hazards.

Each employer shall be responsible for the safe condition of tools and equipment used by employees, including tools and equipment which may be furnished by employees.   On or about December 3, 2025, at an establishment located at 7079 Guelig Road, Lena, Wisconsin, in the South Building, the employer did not to ensure DeWalt air compressor with Ingersoll Rand Type-30 (T30) tank had the pressure regulated to less than 120 psi while providing compressed air to the Everwin PN65M Magnesium Pallet Coil Nailer exposing employees to struck by hazards.

29 CFR  1910.243(b)(2):  Airhose. Hose and hose connections used for conducting compressed air to utilization equipment shall be designed for the pressure and service to which they are subjected.  On or about December 3, 2025, at an establishment located at 7079 Guelig Road, Lena, Wisconsin, in the South Building, the employer did not ensure the hose and hose connections used for compressed air were designed for the pressure and service for which they were subjected to.  Specifically, employees were utilizing an Everwin PN65M Magnesium Pallet Coil Nailer that was connected to the air hose using a worm gear clamp rather than a crimped fitting.

On or about December 3, 2025, at an establishment located at 7079 Guelig Road, Lena, Wisconsin, in the South Building, the employer did not ensure that personnel operating the Yale propane forklifts had been re-evaluated every three years, as required by the standard.
