29 CFR  1910.147(c)(7)(i)(A):Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  On or before November 18, 2025, the employer did not ensure machine operators authorized to perform service and maintenance activities such as cleaning and sanitation of the Yiren, Model XYD-CX Pocket Bread Molding Machine received training in the recognition of the hazardous energy sources of the machine, type and magnitude of the energy available in the workplace, and methods and means necessary for energy isolation and control.

29 CFR  1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  On or about November 18, 2025, the employer did not ensure the point of operation guarding of the Yiren, Model XYD-CX Pocket Bread Molding Machine was designed and/or constructed to prevent operators from having any part of their body in the danger zone during the operation cycle.  The pocket bread molding machine's point of operation guarding had an opening below the point of operation barrier guard large enough for the operator's fingers to reach into and contact the rotating sandwich molds (point of operation) of the machine.
