The employee did not clear a retreat path exposing the employee to stuck by, falling object and crushing hazards.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

The excavator was not properly stabilized as the excavator was not equipped with outriggers or other means of support while on a slope. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
