29 CFR  1904.40(a): The employer did not provide an authorized government representative the records within the four business hours.   a) Establishment- On or about December 8, 2026, the employer did not provide the OSHA 300, 300A, and/or 301 records within four (4) business hours of the request made by an authorized OSHA representative, as required by 29 CFR 1904.40(a). This failure impeded OSHA's ability to conduct a timely evaluation of injury and illness trends and deprived employees of access to required safety and health information. An employee fell from a ladder at heights above 6 feet and sustained serious injuries.
