29 CFR  1910.178(k)(1): The brakes of highway truck(s) were not set and wheel chocks placed under the rear wheels to prevent truck(s) from rolling while they are boarded with powered industrial trucks:  Location: Worksite located at 108 Industrial Drive, Jersey City, NJ 07305; Loading dock area  a) On or about 11/25/2025, the brakes of the truck were disengaged and the employer did not ensure the use of wheel chocks, or similar means, to prevent the truck from moving while employees operated forklifts on the truck flatbed to load/unload materials. The forklift, along with the employee in it, fell approximately 5 feet from the flatbed of the moving truck to the ground below.
