29 CFR  1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  (a) On or about December 8, 2025, the employer did not ensure that employees who perform maintenance on the Dongguan Yasen (model YSTLXR1100-AI1 (CPM)) received training on the hazardous energy control program and procedures for controlling hazardous energy sources.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) On or about December 8, 2025, the employer failed to ensure that the Dongguan Yasen (Model no. YSTLXR1100-AI1 (CPM), machine was equipped with adequate guarding to protect employees from injury while loading the unit, thereby exposing employees to a caught-in/entanglement hazard.
