29 CFR  1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  A) 1037 State St. Perth Amboy, NJ 08861.  On or about December 8, 2025, and thereafter, the employer failed to have a Hearing Conservation Program, including providing an annual audiometric testing and training  to employee(s) exposed to excess noise throughout the facility.    OSHA conducted personal exposure monitoring for noise on December 16, 2025 while employees were conducting normal daily activities for five employees at the manufacturing area. Personal monitoring while operating machine #2 measured exposures to noise at 53 % Dose; equivalent to 8-hour Time-weighted averages (TWA) of 86 dBA.
