29 CFR  1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  (a)  On or about December 9, 2025, at an establishment located at 6155 Mesker Street in Weston, Wisconsin, employees were required to perform maintenance and servicing tasks on the Sawteq S-400, the procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques nor did the procedure identify each type and magnitude of energy, electric magnitude and pneumatic type and magnitude to be de-energized for the control of hazardous energy and steps to verify de-energization.  (b)  On or about December 9, 2025, at an establishment located at 6155 Mesker Street in Weston, Wisconsin, an employee was required to perform maintenance and servicing tasks on the CNC Rover, the procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques nor did the procedure identify each type and magnitude of energy, electric magnitude and pneumatic type and magnitude to be de-energized for the control of hazardous energy and steps to verify de-energization.  (c)  On or about December 9, 2025, at an establishment located at 6155 Mesker Street in Weston, Wisconsin, an employee was required to perform maintenance and servicing tasks on the Doweling Machine, the procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques nor did the procedure identify each type and magnitude of energy, electric magnitude and pneumatic type and magnitude to be de-energized for the control of hazardous energy and steps to verify de-energization.  (d)  On or about December 9, 2025, at an establishment located at 6155 Mesker Street in Weston, Wisconsin, an employee was required to perform maintenance and servicing tasks on the Holzer Edgebander, the procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques nor did the procedure identify each type and magnitude of energy, electric magnitude and pneumatic type and magnitude to be de-energized for the control of hazardous energy and steps to verify de-energization.

29 CFR  1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  On or about December 9, 2025, the Machining Department at an establishment located at 6155 Mesker Street in Weston, Wisconsin, the employer did not provide written periodic inspection certifications for the lockout tag out audits on machines including but not limited to the Sawteq S-400 Beam Saw, Holzer Edge Bander, Doweling Machine, and Rover Router.

29 CFR  1910.213(c)(1):  Each circular hand-fed ripsaw shall be guarded by a hood which shall completely enclose that portion of the saw above the table and that portion of the saw above the material being cut.   On or about November 20, 2025, at an establishment located at 6155 Mesker Street in Weston, Wisconsin, in the Standard Fabrication area, an employee was exposed to a struck-by hazard while ripping boards on the VanGuard table saw from the blade 2 and 1/8 inches above the table without the blade top guard installed.

29 CFR  1910.213(c)(2): Each hand-fed circular ripsaw shall be furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator. The spreader shall be made of hard tempered steel, or its equivalent, and shall be thinner than the saw kerf. It shall be of sufficient width to provide adequate stiffness or rigidity to resist any reasonable side thrust or blow tending to bend or throw it out of position. The spreader shall be attached so that it will remain in true alignment with the saw even when either the saw or table is tilted. The provision of a spreader in connection with grooving, dadoing, or rabbeting is not required. On the completion of such operations, the spreader shall be immediately replaced.  On or about November 20, 2025, at an establishment located at 6155 Mesker Street in Weston, Wisconsin, in the Standard Fabrication area, the employer did not ensure the VanGuard SI400E table saw was equipped with a spreader bar to prevent binding and or kickback prior to an employee operating the saw to rip wooden boards.

29 CFR  1910.213(c)(3): Each hand-fed circular ripsaw shall be provided with nonkickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator. They shall be designed to provide adequate holding power for all the thicknesses of materials being cut.   On or about November 20, 2025, at an establishment located at 6155 Mesker Street in Weston, Wisconsin, in the Standard Fabrication area, the employer did not ensure the VanGuard SI400E table saw was equipped with anti-kickback equipment to prevent potential struck-by hazards prior to an employee operating the saw to rip wooden boards.
