29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Production Floor, Machine 141 - On or about December 3, 2025, and at times prior thereto, an employee was operating a Curioni Flexo/Folder Gluer machine that folds boxes.  The employee was exposed to a moving chain and rollers when cleaning the machine. The employer did not ensure employees kept their hands clear of the chain and rollers with the use of machine guarding methods.
