29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  (a)Wambli Ska dba Sacred Storm Buffalo at 1585 Valley Drive, Rapid City, SD 57703: On or about and at times prior to December 10, 2025, the employer did not ensure that missing electrical equipment was replaced in that an electrical junction box cover plate adjacent to the final station of the Packaging Systems Incorporated - Automated Packaging Machine was not installed. This condition exposed employees to shock and electrocution hazards.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the employer's energy control procedures required by this standard:    (a)Wambli Ska dba Sacred Storm Buffalo at 1585 Valley Drive, Rapid City, SD 57703: On or about and at times prior to December 10, 2025, the employer did not ensure specific energy control procedures to deenergize the ScottPec MADO Cutter - Meat Chopper Machine Supra 70, Model Number: MSM765SL, Serial Number: 113826 were utilized prior to the operator's hand entering the point of operation / danger zone. This condition exposed employees to crushing, amputation, shock, and air embolisms while accessing the equipment.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  (a)Wambli Ska dba Sacred Storm Buffalo at 1585 Valley Drive, Rapid City, SD 57703: On or about and at times prior to December 10, 2025, the employer did not ensure a periodic inspection was conducted at least annually to ensure that its lockout/tagout program was being followed and any deviations or inadequacies were identified. This condition exposed employees to amputation, crushing and shock hazards associated with an improperly locked out equipment.
