On or about December 10, 2025, and before at the worksite located at 2229 NE Town Center BLVD, Lee Summit, MO 64064: Employees were exposed to struck by and crushed by hazards while operating forklifts. Employees were operating forklifts to move materials throughout the facility. The employer did not verify that employees had adequate training before operating fork trucks in the workplace.
