29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about December 8, 2025, the employer did not ensure that the company's energy control procedures were utilized to control mechanical movement of the part clamp on Tap 1 while employees performed unjamming tasks to clear a stuck metal fitting from the clamp, exposing employees to a pinch point hazard.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed: On or about December 8, 2025, in the Tapping and Marking Department, the employer had not certified that the periodic inspection of the energy control procedures for Tap 1 had been performed to verify control of electric and pneumatic energy sources. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.
