29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) New residential construction located at Lot 5 Brooklet Blvd, Dawsonville, GA 30534, Top Plate: On or about 12/15/2025, the employer exposed employees to fall hazards of approximately 10-feet, to the lower level, in that a fall protection system was not provided to employees engaged in framing activities using the top plate as a walking working surface.

29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval:  a) New residential construction located at Lot 5 Brooklet Blvd, Dawsonville, GA 30534: On or about December 15, 2025, the employer exposed employees to potential struck-by hazards while operating a JLG forklift truck. A job-made basket attachment was used for debris handling on this equipment without obtaining written approval from the manufacturer.
