29 CFR  1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  A) Worldpak Inc/1037 State Street, Perth Amboy, NJ 08861. The employer did not ensure that the Ro-An Polystar machines were guarded to protect against injuries.  The machines did not have any guarding to prevent employees from accessing the zone of danger.   Condition observed on or about 12/16/2025.
