29 CFR  1904.29(b)(1):A log of all recordable work-related injuries and illnesses (OSHA 300 or equivalent), was not completed in the detail as required by the regulation:  a.)  U.S. Postal Service - Erie, SCD, 2108 E. 38th Street, Erie, PA 16515 - The OSHA 300 Injury and Illness log did not have the required information in Column F.  Multiple cases in Calendar Year 2025  did not record the required information, including, but not limited to, the object/substance that  injured the employee, the part of the body effected, and/or  the injury.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

U.S. Postal Service - Erie, SCD, 2108 E. 38th Street, Erie, PA 16515 - On or about 12/16/25, and at times prior thereto, the fire alarm system at the facility was not operational.  The alternative measures were not effective for the fire alarm system.    ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.165(b)(1): The employee alarm system did not provide warning for necessary emergency action as called for in the emergency action plan, or for reaction time for safe escape of employees from the workplace or the immediate work area, or both:  a.)  U.S. Postal Service - Erie, SCD, 2108 E. 38th Street, Erie, PA 16515 - On or about 12/16/25, and at times prior thereto, the Emergency Action Plan had not been changed to reflect the use of the air horns.  The main alarm system had not been operational.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

U.S. Postal Service - Erie, SCD, 2108 E. 38th Street, Erie, PA 16515 - On or about 12/16/25, and at times prior thereto, the air horns were being used as an audible alarm while the primary alarm system was not operational.  The air horn alarm system was not effective for hearing impaired occupants.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.165(e): Manually operated actuation devices for use in conjunction with employee alarms were not unobstructed, conspicuous and readily accessible:  a.)  U.S. Postal Service - Erie, SCD, 2108 E. 38th Street, Erie, PA 16515 - On or about 12/16/25, and at times prior thereto, air horns had been stationed at the fire alarm system pull boxes with signage of the fire alarms not being operational.  The signs remained on the pull boxes but the air horns were moved to the supervisor's desks.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
