29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. (a) On the roof of the building - On or about December 17, 2025, the employer exposed an employee to a fall hazard, in that an employee used a lifeline wrapped around a single roof truss in lieu of installing an anchor designed to support at least 5,000 pounds.

29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. (a) On the Northeast corner of the building - On or about December 17, 2025, employees were exposed to a fall hazard, in that the employees used an extension ladder for access to the second floor that did not have its side rails extended at least 3 feet above the upper landing surface with a 13' fall to ground distance.
