29 CFR  1926.451(d)(3)(x): Single tiebacks were installed at an angle:   (a): Exterior Suspended Scaffold South Side - Four employees working from the suspended scaffold were exposed to scaffold collapse hazards. The suspended scaffold beam tiebacks were installed at an angle, on or about December 11, 2025.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee   (a): Exterior Suspended Scaffold South Side - Four employees were using personal fall arrest anchor points that were not independent of the suspended scaffold tieback anchors. The employees were exposed to fall hazards of approximately 30-60 feet when working on the suspended scaffold with improper anchor points, on or about December 11, 2025.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
