29 CFR 1910.106(h)(4)(iv)(a): Equipment was not designed and arranged to prevent the unintentional escape of liquids and vapors and to minimize the quantity escaping in the event of accidental release.  On or about 11 December 2025, employees of Mid-West Industrial Chemical Co., located at 1509 Sublette Avenue in St. Louis, Missouri, were exposed to fire hazards when using equipment that was not designed or arranged to prevent the unintentional escape of liquids and vapors. An employee was burned when flammable liquid splashed on the employee and later ignited.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective.  On or about 11 December 2025, employees of Mid-West Industrial Chemical Co, located at 1509 Sublette Avenue in St. Louis, Missouri, were exposed to electrical and fire hazards from grounding that was not permanent, continuous, and effective. A Sears Craftsman Drill Press being used to mix flammable liquids did not have a grounding pin.

On or about 11 December 2025, employees of Mid-West Industrial Chemical Co, located at 1509 Sublette Avenue in St. Louis, Missouri, were exposed to electrical and fire hazards by utilizing equipment, wiring methods, and installations of equipment in hazardous locations that were not intrinsically safe. A Sears Craftsman Drill Press was being used to mix Category 2 flammable liquids.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment or whenever a new hazard that the employees had not been previously trained about was introduced into their work area.  On or about 11 December 2025, employees of Mid-West Industrial Chemical Co., located at 1509 Sublette Avenue in St. Louis, Missouri, were exposed to fire hazards in that employees were not provided effective information and training on hazardous chemicals in their work area. An employee unknowingly mixed flammable liquids in equipment that was not intrinsically safe causing it to ignite.
