29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a) New single-family residential construction located on 5205 Mulberry St. Flowery Branch, GA 30542, Second floor top plate: On or about 12/18/2025, the employer exposed employees to struck-by hazards in that, the employer failed to ensure that protective eyewear was provided to employees engaged in framing activities while operating pneumatic nail guns.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) New single-family residential construction located on 5205 Mulberry St. Flowery Branch, GA 30542, Second floor top plate: On or about 12/18/2025, the employer exposed employees to fall hazards of approximately 8-feet, 8-inches to the inside of the structure, and 24-feet, 3-inches to the outside of the structure, in that a fall protection system was not provided while engaged in framing activities.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) New single-family residential construction located on 5205 Mulberry St. Flowery Branch, GA 30542,  Second floor: On or about 12/18/2025, the employer exposed employee to a fall hazard of approximetly 8-feet 8-inches to the lower ground below, in that the portable extension ladder used to access the upper working area did not have its rails extending at least three feet above the upper landing area.
