On or about December 20th, 2025, at a worksite located at 25111 East 175th Street Pleasant Hill, MO 64080, employees were exposed to electrical hazards that include, but are not limited to, electrocution, electric shock, arc flash, electric burns and electric fire. The employer failed to provide regular supervision during electrical-related work practices that include, but are not limited to, opening and closing 4160 volt switchgear breaker assemblies and performing work on the assemblies, to ensure the employees complied with all safety related work practices. The employer routinely assigned tasks to IC&E Technicians involving electrical equipment without ensuring they complied with all safety-related work practices.

29 CFR 1910.269(c)(5):An employee working alone need not conduct a job briefing. However, the employer is required to ensure that the tasks to be performed are planned as if a briefing were required. On or about December 20th, 2025, at a worksite located at 25111 East 175th Street Pleasant Hill, MO 64080, employees were exposed to electrical hazards that include, but are not limited to, electrocution, electric shock, and arc flash. The employer exposed the employees to these hazards by failing to ensure that the tasks associated with working on or near live electrical equipment located inside 120 volt and 4160 volt breaker cabinets were planned as if a job briefing were conducted. Aspects associated with job briefings that include but are not limited to, hazards related to the work, personal protective equipment requirements, energy source controls, and special precautions to be taken, were not planned prior to employees performing work on the breaker cabinets.

29 CFR 1910.269(l)(5)(ii):When an employee performs work near exposed parts energized at more than 600 volts, but not more than 72.5 kilovolts, and is not wearing rubber insulating gloves, being protected by insulating equipment covering the energized parts, performing work using live-line tools, or performing live-line bare hand work under paragraph (q)(3) of this section, the employee must work from a position where he or she cannot reach into the minimum approach distance, established by the employer under paragraph (l)(3)(i) of this section. On or about December 20th, 2025, at a worksite located at 25111 East 175th Street Pleasant Hill, MO 64080, employees were exposed to electrical hazards that include, but are not limited to, electrocution, electric shock, arc flash and electric burns. The employer exposed IC&E technicians to these hazards by failing to prevent them from working from a position where they could reach into the minimum approach distance associated with the 4160 volt breaker cabinets during troubleshooting and maintenance activities without the use of such protective measures as rubber insulating gloves.
