29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  105 Krameria Street, Denver, Colorado: On or about December 23, 2025, the employer failed to provide a safe means of egress from a trench that measured approximately 13 feet in depth. Employees installing utility piping worked in a trench without a secured, properly placed ladder. This condition increased the risk of entrapment and delayed escape during an emergency, exposing employees to cave-in hazards.

29 CFR  1926.651(e): Employee was not prohibited to be underneath loads handled by lifting or digging equipment:  105 Krameria Street, Denver, Colorado: On or about December 23, 2025, the employer did not ensure employees were kept clear of loads suspended by lifting or digging equipment. Employees installing a pipeline worked inside a trench shield while it remained attached to the excavator via lifting rigging. This exposed the employee to struck-by, crushed-by, and caught-in/between hazards if the shield shifted, was lifted, or if rigging failed.

29 CFR  1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures:  105 Krameria Street, Denver, Colorado, at 80220: On or about December 23, 2025, the employer failed to provide necessary measures to maintain the stability of street traffic while excavating a trench to install sewer lines. The trench was dug on paved road without underpinning or other traffic support systems to protect the structure from loss of soil support. This exposed employees working in and around the trench to struck-by, crushed-by, and caught-in hazards from potential settlement, shifting, or collapse of the adjacent road or its foundation.
