(a)  In front of the building - On or about December 23, 2025, the employer exposed employees to struck-by and crushed-by hazards, in that each employee did not wear the seatbelt while operating a Sky-Trak 6034 forklift.

29 CFR  1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  (a) In front of the building - On or about December 23, 2025, the employer exposed employees to fall and struck-by hazards, in that a wooden pallet used as a scaffold was moved vertically and horizontally and was not attached to the forks, with a 13' fall to ground distance.
