29 CFR  1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  a) On or about December 30, 2025, and at times prior, the employer did not ensure that a portable ladder used to access the 2nd story of the residential home to conduct solar panel installation was extended at least 3 feet above the landing surface or secured.

29 CFR  1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed.  a) On or about December 30, 2025, and at times prior, the employer did not ensure that an A-frame ladder used to access the 2nd story of the home from the first story roof was used for the purpose for which it was designed, in that it was not opened, but leaned up against the side of the house.
