29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: a) On or about December 30, 2025, and at times prior, the employer did not ensure that a portable ladder used to access the 2nd story of the residential home to conduct solar panel installation was extended at least 3 feet above the landing surface or secured.
