29 CFR  1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about November 14, 2025, employees performing roofing operation at a residential property were not trained to recognize and minimize falls, exposing employees to a 20 foot fall.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.1053(b)(22):An employee shall not carry any object or load that could cause the employee to lose balance and fall.  On or about November 14, 2025, an employee using an extension ladder was carrying roofing material causing the employee to lose balance and fall,  exposing employee to a fatal 20 foot fall.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.  On or about November 14, 2025, employees required to use extension ladders were not trained to recognize and minimize the hazards associated with ladder use, exposing employees to a 20 foot fall.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1904.39(a)(1):Basic Requirement.  Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor.  On or about November 14, 2025,  the employer failed to report within 24-hours a work-related fatality that resulted in the death of one employee.  No abatement documentation or certification  is required of this item.
