29 CFR 1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about November 14, 2025, employees performing roofing operation at a residential property were not trained to recognize and minimize falls, exposing employees to a 20 foot fall. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about November 14, 2025, an employee using an extension ladder was carrying roofing material causing the employee to lose balance and fall, exposing employee to a fatal 20 foot fall. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
