29 CFR  1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) Dimitri W Plumbing - Worksite located at:  3044 Joshua Rd, Lafayette Hill, PA - On or about December 10, 2025, employees were installing and connecting piping down in a trench that was approximately 7 feet 4 inches deep.   Employees were not wearing hard hats to protect from the possible danger of struck-by head injuries from adjacent unsafe spoil piles.  No abatement certification or documentation required.

The excavation and adjacent areas were not inspected by a competent person in accordance with this section.   No abatement certification or documentation required.

The employer was not using a ladder that extended at least 3 feet above the landing surface in accordance with this section.   No abatement certification or documentation required.
