29 CFR  1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  On or about January 7, 2026, at an establishment located at 300 S Krueger Street, Suring, WI, the employer did not conduct periodic inspections of their energy control procedures to ensure that the procedures and the requirements of this standard were followed. Employees performed servicing and maintenance tasks on equipment including but not limited to, Cencateq, Vantech 510, Homag Edge Bander, and the Time Saver Sander.

29 CFR  1910.212(a)(2):General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.  On or about January 7, 2026, at an establishment located at 300 S Krueger Street, Suring, WI, employees operating the Pallet Stock Cut Off Saw were exposed to potential caught-in hazards when the tunnel guards and blade guard were installed in such a way that they created a pinch point hazard.
