29 CFR 1926.28(a): The employer failed to require employees to wear appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions.   a. At 1375 US Highway 1 South, Saint Augustine, Florida, on or about January 6, 2026, the employer failed to ensure that employees performed an adequate hazard assessment identifying all hazards associated with the task being performed and required personal protective equipment to protect against such hazards. As a result, employees were exposed to fall hazards posed by unprotected skylight openings.
