(a) At the MCC for line #4 starter in paper mill #2 - On or about December 30, 2025, and at times prior, employees were exposed to electrical shock and arc flash hazards, in that employees working on an open 480V MCC (GE/ABB model #8000, Serial #559X0839L02) did not distinguish exposed live parts from other parts of electric equipment.
