29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about January 8, 2026, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at 718 Thoreau Ave, Akron Ohio 44306.  The employees were using a battery powered saw, air powered nail guns, and hand tools  and the employer did not ensure that the employees were wearing protective safety glasses.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about January 8, 2026, employees were exposed to a fall of approximately 19.3 feet while performing roofing activities on a residential home located at 718 Thoreau Ave, Akron Ohio 44306. The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.  Alvarado Exterior Solutions, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFE 1926.501(b)(10), which was contained in OSHA inspection number 1688251,  citation number 1,  item number 1, and was affirmed as a final order on 9/22/2023, with respect to a workplace located at  Corner of Bagley and Lewis RD, Olmsted Falls Ohio 44138.
