29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section:  (a) Above south overhead door, On or about January 7, 2026, the employer did not ensure the 180-degree exposure guard was installed on a Milwaukee brand cordless angle grinder (S/N L64BF2418 13039) equipped with an abrasive grinding wheel, while an employee used it to grind the heads off of rivets.  (b) Above west overhead door, On or about January 7, 2026, the employer did not ensure the 180-degree exposure guard was installed on a Milwaukee brand cordless angle grinder (S/N L64BF2418 13039) equipped with an abrasive grinding wheel, while an employee used it to grind the heads off of rivets.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) Northeast side of roof; On or about January 7, 2026 and dates prior, the employer did not ensure that each employee was protected from fall hazards of more than 40-feet above the ground during standing seam roof panel removal operations on a Butler style steel building.
