29 CFR  1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a)  On or about December 30, 2025, employees working around the Tray-Matic tray former (Model No. SWFIT4 and Serial No. 4656) were exposed to an amputation hazard when employees entered the hazard zone / point of operation through the material discharge shoot. The  point of operation(s) was not properly guarded.
