29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  a) On or about January 12, 2026 - On lot 211 Magnolia Court, Sewickley, PA 15143, employees were using a portable extension ladder to access the first floor of a 2-story residential construction project. The ladder side rails did not extend at least 3 feet above the upper landing surface. Employees using the ladder to access the first floor were exposed to a fall hazard of approximately 10-14 feet to the ground level.

29 CFR  1926.1060(b): Retraining shall be provided for each employee as necessary so that the employee maintains the understanding and knowledge acquired through compliance with this section:  a) On or about January 12, 2026 - On lot 211 Magnolia Court, Sewickley, PA 15143, employees utilizing a portable extension ladder for residential framing activities at heights up to 14 feet were not trained on proper ladder usage. Employees using the ladder to access the first floor of the home did not extend the ladder at least 3 feet above the upper landing surface.
