29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  (a) On January 7, 2026, employees used a damaged extension cord to provide 120v to a SKILSAW SPT77WML-01 7-1/4 inch, 15-amp worm drive circular saw being used for framing activities.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) On January 7, 2026,  employees were exposed to falls of approximately 16 feet and 8 inches while performing framing activities.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On January 7, 2026, employees used an extension ladder to access trusses while performing framing activities and were exposed to falls of approximately 16 feet 8 inches. The extension ladder extended approximately 1 foot past the landing surface.
