29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  a) Facility?Wide - On or about January 13, 2026, the employer did not ensure that all passageways were kept clean and orderly. Flexible extension cords were lying across walking paths throughout the facility. Employees using these passageways were exposed to tripping hazards due to the presence of the cords.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a) Facility-wide - On or about January 13, 2026, at the facility-wide location, employees were exposed to electrical hazards of 120 volts when flexible cords, including extension cords, were used as a substitute for permanent (fixed) wiring to power multiple grinding tools and other hand?held powered equipment. This condition exposed employees to electrical and fire hazards.
