29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access: A) 30 Locust Avenue, Scarsdale - NY: Employees were exposed to fall hazards of approximately 22-feet while using an extension ladder to access an upper level of a structure. The extension ladder that was used, was not extended at all over the level that they were accessing and not 3-feet above the landing which is required by the manufacturers instructions. The employees were exposed to falls, ladder collapse hazards, on or about, December 10th, 2025.
