USS Mustin: On January 13, 2026, at 11:15am the Compliance Safety and Health Officer (CSHO) requested OSHA Form 300 and 300A for calendar year 2025. The employer did not provide the Form 300 for calendar year 2025 and the 300A for calendar year 2025 to the CSHO within the four business hours.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:  USS Mustin, 1-378-2-Q, SS GTRB Gen #3 Exhaust Trunk: On or about January 13, 2026, and at times prior thereto, employee(s) used a half face respirator and did not store it to protect it from damage or dust. This condition potentially exposed employee(s) to respiratory hazard(s).
