29 CFR  1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about January 14, 2026, employees performing carpentry work were not protected from a fall, exposing employees to falls of 13-19 feet.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.502(d)(6)(i): Unless the snaphook is a locking type and designed for the following connections, snaphooks shall not be engaged directly to webbing, rope or wire rope.  On or about January 14, 2026, employees connected snaphooks directly to a wire rope (horizontal lifeline), exposing employees to hazards associated with gate failure, cross-loading and or catastrophic failure.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.502(d)(8): Horizontal lifelines shall be designed, installed, and used, under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintains a safety factor of at least two.  On or about January 14, 2026, employees used a horizontal lifeline that was not inspected by a qualified person or under the supervision of a qualified person, exposing employees to the hazards associated with using a horizontal lifeline systems improperly.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about January 14, 2026, employees performing carpentry work at heights exceeding six feet were not trained to recognize and minimize fall hazards, exposing the employees to falls between 13-19 feet.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
