On or about January 15, 2026, at 2191 Rider rain Lane Apopka Florida 32703, employees were exposed to fall hazards of 20 feet while preparing to installing solar panels on a 6:12 pitch roof, in that, fall protection was not used.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a. On or about January 15, 2026, at 2191 Rider rain Lane Apopka Florida 32703, employees were exposed to fall hazards of 20 feet while climb on and off roof landings in that the side rails of the ladder extended 12 inches past the roof landing.
