29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) At or near 50 Johnson Ave., Englewood Cliffs NJ - Roof: employee without any means of fall protection was exposed to approximately 20 feet fall hazard, while removing decoration lights from the edge of the roof of a residential building, on or about 1/15/26.

29 CFR  1910.67(c)(2)(v): 29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements of subpart I of this part were not worn or attached to the boom or basket when working from a bucket truck.   a) At or near 50 Johnson Ave., Englewood Cliffs NJ - Bucket Truck - Front of Building: employee without any means of fall protection was exposed to approximately 20 feet fall hazard, while utilizing the bucket of a truck to reach the roof of a residential building, on or about 1/15/26.  b) At or near 50 Johnson Ave., Englewood Cliffs NJ - Bucket Truck - Front Garage Doors: employee without any means of fall protection was exposed to approximately 20 feet fall hazard, while utilizing the bucket of a truck to remove decoration lights from the edge of the roof of a residential building, on or about 1/15/26.
