29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about January  1, 2026, the employer did not establish an energy control program consisting of energy control procedures, employee training, and periodic inspections. Employees utilize machinery to include but not limited to: powder mill and heat exchanger/pasteurizer systems. Employees perform activities such as set up, change over, and service/maintenance activities requiring the isolation of energy sources including, electrical, mechanical, thermal, gravity,  and pneumatic.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program which at included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):  On or about January 16, 2026, the employer did not ensure that a written hazard communication program was developed or implemented. Employees were exposed to fire, health, reactivity, and contact hazards when utilizing hazardous chemicals including, but not limited to: Chlorine, Sodium Hypochlorite 12.5%, CFT Superfoam, Defoamer EG, AlpetD2 Sanitizer, and Hydragenix AHF.
